Population health managers constantly focus on attaining positive outcomes for their various and populations. Risk segmentation and stratification provide useful steps in aligning program interventions with consumers’ needs and wants. Health care service delivery gaps often occur during transitions of care and/or specialized care. Each of the three vignettes suggest a need for a quality improvement initiative. Choose between the diverse quality improvement options covered in this unit for each of the four vignettes and then briefly outline a rationale for your decision (1-2 pages). Assume you are the population health manager in each of these situations.
