By-standers dialled 999 and she was taken to hospital. She was assessed in the Emergency Department. It had been confirmed that Janice has been diagnosed with a left middle cerebral artery ischaemic stroke. She has a right hemiparesis and aphasia.

Patricia Burns is a 69-year-old woman who lives with her husband William aged 71 years’ old who has been diagnosed with dementia. They have two grown up children and their daughter Gail lives around the corner. Patricia smokes 15 cigarettes a day, she is overweight with a BMI of 35. In addition, she has hypertension that is controlled by medication and a history of recurrent urinary tract infections. She has been feeling more tired lately and experienced shortness of breath on minimal exertion. Her daughter has commented that she is looking pale all the time. After the investigations Patricia has been diagnosed with Chronic Kidney Disease she has met with the consultant and the renal nurse to discuss her options for management of her condition. They have explained that she will need to have dialysis to support her kidney function as it is inadequate. They will also give her advice on changes that she will need to make to her diet and fluid intake plus changes to her lifestyle.
