1) Using the template provided you are asked to write a risk of serious harm summary on a case you are supervising. The summary should draw on the literature that informs your risk assessment. This part will be 1000 words (+/-10%) excluding the template headings. (please find attached template & guidance for Risk Of Serious Harm)
2 Using a model of reflection, provide a critical reflection on undertaking the risk summary task. This should reflect on your learning and consider how this will inform future practice. This will be 500 words (+/-10%)
Guidance. Part 1
Part 1:
This assignment will be based on your risk assessment of someone with whom you are working. They do not need to be formally assessed as high risk, but should present some concerns in relation to risk of harm.
The person can be someone you are co-working with another practitioner.
Please use the template provided here to undertake this assessment
Topics in the Moodle module on ‘Analysing risk and need’, ‘The tools of the trade’, ‘The Art of Decision Making’ and ‘Critical risk assessment of specific offences’ will be particularly useful in formulating ideas for this task.
You are not required to provide a pen picture, however, the summary should include sufficient details of the individual case throughout to enable the marker to form a picture of the case – eg, circumstances of the offending, pattern of behaviour, characteristics of the individual including diversity/marginalisation
The risk summary should provide the rationale for your assessment
It will be necessary to research risks associated with the specific behaviour that is presenting the risk of serious harm e.g.: domestic abuse, serious group offending, sexual offending, etc.
When considering the imminency of risk you should draw on both actuarial and dynamic risk assessment tools and critically discuss how they inform your professional judgement
You need to critically examine the dynamic risk factors associated specifically with your case whilst drawing on the supporting literature
There should be an appreciation of models of rehabilitation when considering how risk may be reduced (e.g. desistance theory, GLM, RNR, strengths based approaches)

Part 2:
When undertaking the reflective task on the risk of serious harm activity please ensure that you use a model of reflection. This link provides an overview of a number of reflective models. This is not an exhaustive list and you are not restricted to what is contained here, use any model that works for you.
As this task is only 500 words you may wish to focus on one element of learning from the experience, for example, the impact of diversity/marginalisation on your risk assessment, reflection on your personal bias, using your professional discretion when not in agreement with an actuarial score.
The following questions are helpful pointers for the reflective task, however they are not exhaustive:
How did the exercise feel to do? E.g. did it challenge any thoughts/values you have?
What new knowledge did you gain?
Consider the extent to which you use the evidence base (e.g. literature/research) in your decision making
How will this inform your practice in the future?
Do not use the subheadings of your chosen reflective model in your writing.
First person language is encouraged when you are exploring your experience, values and emotions.
You are expected to identify the key authors who have identified your approach or thinking in both parts of this task. References are therefore required.
