Scenario
Public and private payers have decided to roll out a Patient Centered Medical Home (PCMH) program, and your hospital system has agreed that their three clinics will participate.
As part of the program, reimbursement will no longer be based solely on the number of services provided. Instead, the providers will be reimbursed based on quality metrics and outcomes and cost containment.
Care coordination ensures patients are compliant with treatment plans and that any care provided by a specialist or facility is cost-effective.
As the HIM Director, you have been named Committee Chair for the team tasked with developing the database intended to gather all pertinent data to monitor the program.
You are meeting with the committee, including a data modeler, next week to discuss the data components required to monitor the changes made within the practices.
